By submitting this form, you are REQUESTING to cancel. This does not automatically cancel your account. We will contact you if you are not eligible for cancellation.
The one month notice applies for all eligible cancels- you will receive one final bill after the form is submitted.
Annual fees are to be paid if the date of your final bill is the same as your fee date .
Members must fulfill their contract before cancelling their account. If you are still in a commit, please freeze your account if you are not ready to return.
All cancel requests are to be approved by the manager. If you have moved more than 25 miles away, you must submit proof of move if you are in a contract. If you are cancelling due to medical reasons, we must receive a doctors note if you are in a contract.
